[Tramadol Hydrochloride]
Description / Properties
Tramal is an opioid analgesic agent. Each tablet, capsule and injection contains tramadol hydrochloride.
Indications
For the treatment of moderate to severe acute and chronic painful conditions.
Contraindication
Hypersensitivity to the active ingredient, intoxication with alcohol, hypnotics, centrally acting analgesics, opioids or psychotropics, or together with MAOIs or within two weeks of their withdrawal, pregnancy and lactation.
Drug Interactions
There are no restrictions on the concomitant use of tramadol with peripheral analgesics or NSAIDs. Cimetidine inhibits the metabolism of opioids. But this interaction is not considered clinically significant. Carbamazepine enhances the rate of metabolism of tramadol. An increase in dosage or reduction in dosage intervals may be necessary. Tramadol must not be co-prescribed with monoamine oxidase inhibitors because it would carry the risk of enhanced monoaminergic effects.
Side Effects
Nausea, vomiting, drowsiness/somnolence, tiredness/fatigue, dizziness, headache, confusion, hallucinations, diaphoresis and infrequently respiratory depression.
Warning and Precautions
May cause drowsiness, especially with CNS depressants or alcohol. Not suitable for opioid dependent patients. Caution in patients with head injury, increased intracranial pressure, convulsive disorders, severe hepatic/renal impairment, respiratory depression or in shock. Caution when used with centrally acting agents, carbamazepine.
Dosage
Depending on severity of pain, 50-100mg not more frequently than 4 hourly; 50mg start for pain associated with chronic conditions. SR tablets: Initial dose 100mg bd, titrate as necessary to 200mg bd. Treatment of chronic conditions should be limited and intermittent and where there exists a medical need.
